W-9 Completion and 1099-Misc Informational Returns

i2E, Inc. requests a W-9 from each Love’s Cup participant in order that we may report awards to the IRS as required by law.  If i2E, Inc. issues a check for $600 or greater, we are required to provide you with a 1099-Misc Informational Return by January 31, 2020.

Write all information legibly in ink.  The address on your W-9 should be your permanent address.  This is especially important if you are moving at the end of the semester, graduating and relocating, or any other reason that might cause you to move.  We want to be sure your 1099 reaches you in a timely manner.  If you do move, please file a change of address with the U.S. Post Office.  A change of address is valid for one year from the date you file it.  Not receiving the form does not take away your liability for reporting and paying any tax due.

The address entered on the form below does NOT have to match the address on your W-9.  This is the address you would like your check mailed the 1st week of May.  If your address below is incorrect, payment could be delayed up to a month.
